Please select “Allergy Testing & Treatment” or “Extended Allergy Testing & Treatment” when scheduling for allergies/sensitivities.
If you need treatment for one allergy or sensitivity, please select “Allergy Testing & Treatment.”
If you need treatment for multiple allergies or sensitivities, or you have unknown allergies, please select “Extended Allergy Testing & Treatment.”
Franklin Acupuncture & Herbal Medicine does not provide treatment for any allergen in which the patient has experienced an anaphylactic shock reaction to. If you have questions about our policy pertaining to anaphylactic shock, please contact the office.
Please select “Functional Medicine Visit” if scheduling for an issue not pertaining to allergies/sensitivities.
A non-refundable deposit of $150 is required to book an Allergy Testing & Treatment or Functional Medicine appointment. This amount will be applied to the total cost ($550) at the time of the appointment. Allergy Testing & Treatment or Functional Medicine appointments that are cancelled at any time for any reason will forfeit the deposit of $150.
A non-refundable deposit of $225 is required to book an Extended Allergy Testing & Treatment appointment. This amount will be applied to the total cost ($775) at the time of the appointment. Extended Allergy Testing & Treatments that are cancelled at any time for any reason will forfeit the deposit of $225.
Allergy Testing & Treatment, Extended Allergy Testing & Treatment, or Functional Medicine appointments can be rescheduled up to 72 hours prior to the scheduled time without having to provide another deposit. Allergy Testing & Treatment or Functional Medicine appointments rescheduled with less than 72 hours notice are subject to another deposit. Appointments rescheduled or cancelled less than 24 hours prior to the scheduled appointment time will be charged the remainder of the appointment cost.
